Request a Repeat Prescription form

If your pet has been seen in a consultation at Woodward Veterinary Practice,  we can issue repeat prescriptions for most drugs. Please fill in the fields below and we will contact you by phone or email to confirm that we can supply the item or items requested. Please allow 48 hours for us to be able to process the request as all prescription items need to be checked by a qualified vet. We will then have items available for you to collect at any time during our opening hours. All prescriptions must be paid for at the time of collection.

If the item requested is not suitable as a repeat item, if your pet has not been seen by us, if your pet needs to be re-examined before the request can be allowed or if the item is unavailable from the manufacturer; we will contact you to make further arrangements.  Please note: it is a legal requirement that we can only supply prescriptions for animals that are "under our care", which means that we must have seen them in the past 6 months. If we have not seen your pet in the past 6 months then a presciption check up will be required before we can give you any more treatment.
For some conditions, this time interval may be less than 6 months.
