Please supply the following information:
- Your name & address
- Your pets name
- The name and strength of each drug required, along with
the dose rate (eg: prednisolone 5mg; 1 tablet twice daily)
- The surgery that you want to collect from (Ashby or Woodville)
- A contact number in case we need to discuss the prescription
with you
If you have a lot of things on repeat prescription, it may
be worth saving a copy of the e-mail for future use.

Please note:
it is a legal requirement that we can only supply prescriptions
for animals that are "under our care", which means
that we must have seen them in the past 6 months. If we have
not seen your pet in the past 6 months then a presciption check
up will be required before we can give you any more treatment.
For some conditions, this time interval may be less than 6 months. |
